PATIENT INFORMATION SHEET
WELCOME
The Board and Staff of the Rehabilitation Center for
Children and Adults welcome you to our Center. While you
are with us, we assure you of our utmost concern for
your welfare. We hope you will be satisfied with our
services and derive many benefits from the treatment you
receive. The following information has been prepared to
answer your questions about our program. Please do not
hesitate to ask for additional information at any time.
ORGANIZATION, PURPOSE & SCOPE
The Rehabilitation Center for Children and Adults, Inc.
is a private, not-for-profit organization whose purpose
is to improve people’s physical function, communication
and independence. The Center’s staff is involved in
ongoing program evaluation, which promotes efficient and
effective patient care. The Center is a not-for-profit
501(c)(3) organization supported by donations.
ADMISSION & DISCHARGE CRITERIA
Therapy may be by self-referral or by a physician
referral. Any patient with a physical or speech
disability, regardless of age or financial status, may
be admitted for services. Discharge occurs when therapy
goals are met or the maximum benefit has been achieved
from skilled therapy services.
FEES
Fees are based on audited costs and are established by
the Board of Governors Budget Committee.
Third parties, including Medicare, medical, liability or
automobile insurance, are billed when applicable.
Individuals needing fee adjustments may make
arrangements with their Program Manager by providing
appropriate documentation. Self & co-pays are due at the
time of treatment.
NOTICE OF EXCLUSIONS FROM
MEDICARE BENEFITS (NEMB)
Medicare may pay for your therapy. Medicare does not pay
for maintenance treatment. If you request therapy beyond
Medicare coverage, you may continue by self-paying. For

HOURS OF OPERATION
The Rehabilitation Center is open 8:00 AM-4:30 PM Monday
through Friday, except for Holidays: New Year’s Day,
Memorial Day, Independence Day, Labor Day, Thanksgiving,
Christmas Eve and Christmas Day. The Center is closed
during a HURRICANE WARNING for the Palm Beach area.
SERVICES AVAILABLE
Services are provided by persons qualified according to
the standards of their state licensing board. Most
therapy is on an individual basis. Physical,
Occupational and Speech Therapy and Preschool activities
are the outpatient services available. A Program Manager
is assigned to each person served and is responsible for
their coordinated treatment. Persons served are
encouraged to actively participate in decisions
regarding their individual goal planning, prioritizing,
implementation and evaluation of therapy by direct
communication with their therapists. We reserve the
right to refuse treatment to anyone who is intoxicated,
obviously under the influence of drugs, or deemed
abusive or behaving unreasonably.
SAFETY OF PATIENTS & PRIVACY NOTICE
Children and patients requiring assistance should be
escorted by a companion while in the building.
Companions or drivers should remain on the premises
while the patient is being treated. A staff member
should be present when patients are in treatment areas.
Human rights, dignity, health, safety and well-being are
promoted for all persons being treated with the same
consideration, courtesy and respect. Prohibited are acts
of abuse (physical, psychological, sexual), corporal
punishment, exploitation, fright, humiliation and
neglect. Patients have the right to all considerations,
freedom of choice, confidentiality and information
regarding their care. A Code of Ethical Conduct promotes
and maintains the highest standards and addresses
procedures for allegations of violations. Physical,
electronic and procedural safeguards protect patient
information.
ABSENCES/CANCELLATIONS/PROMPTNESS
Arriving late or unnecessarily canceling a scheduled
appointment may deny treatment to another patient. After
3 cancellations without notice, therapy will be
discontinued until referred again by the physician.
